Human in the loop

Human in the loop means a person reviews or approves what an automated system produces before it takes effect, with the system designed around that review rather than treating it as an optional extra.

The design question it answers

Not whether to trust the system, but where to put the person. Reviewing everything defeats the purpose; reviewing nothing means the first serious error is discovered by a customer. The useful answer is usually confidence-based: route the uncertain cases to a person and let the clear ones through.

What makes review real rather than theatrical

The reviewer must be able to see why the system produced what it did, and must have time to look. An approval queue nobody can keep up with becomes a button people press, which is worse than no review at all because it manufactures accountability without providing it.

Wherever being wrong is expensive and reversible only with effort — payments, contracts, anything customer-facing, anything that writes to a record of account.
